TERMS & CONDITIONS

Elevate Physio & Performance is a committed healthcare provider aiming to provide quality care for our clients. 
To assist us to better support your health and well-being, please note all service bookings are subject to the below policy:
  • 24 hours notice is required for all cancellations and reschedules. Please contact Elevate Physio & Performance, or your Elevate treating practitioner, directly via the details below for any appointment changes. Cancellations within 24hrs will incur Cancellation fee of the total cost of your appointment. Should this fee be charged, please pay within 7 days. Further appointments cannot be accommodated until this is paid.
  • If you are running late please let us know. If you arrive late but within your appointment time, we will happily still see you for a shortened consultation. We are unfortunately unable to extend your appointment time out of respect for other clients.
  • New clients are required to complete a New Client Form. Please contact us should this form not become available to you during the booking process. 
  • All accounts are required to be paid in full on completion of your appointment. HICAPS is available at reception for instant private health claiming. If you are a Sista Fitness Member, Elevate Physio & Performance services cannot be billed to your gym membership/account.
  • In the event you are unable to process a private health claim on completion of your appointment, we will provide you with a paid invoice to claim directly from your insurer. Please note an Elevate physio & Performance staff member will need to provide this to you, Sista Fitness reception are unable to provide you with an itemised receipt for claiming. 
  • Please note Remedial Massage Services cannot be conducted on clients who have received a COVID-19 Vaccination within 48hours of your appointment.
  • If any changes to your medical profile occur, including pregnancy, recent diagnoses, or medication changes, it is vital you inform your practitioner so they can ensure each treatment is appropriate for you.

We do understand life is full of unforeseen events and circumstances change, so please contact us as soon as possible should you have any difficulty attending your appointment. Our team will also provide you with their direct contact details for ease of communication while under their care.
